Forman Center Terms of Use

Renter Responsibilities

As the Renter, you are responsible for:

1.  Paying the rent in a timely manner
2.  Setting up and putting away all tables, chairs and equipment used, unless you have requested that we do this for you for a fee.
3.  Seeing that the event is conducted in a manner that is responsible and respectful of the Forman Center property and its neighbors
4.  Seeing that the Center is cleaned properly after the event, including:
-Cleaning the floors, counters and all areas used
-Taking out garbage to the dumpster in the back of the center
-Check rest rooms lights and toilets
     -Any other general clean-up
5.  Returning all thermostats to their automatic setting
6.  Turning off lights and lock doors
7.  Returning the key promptly

Failure to do any of these can cause you to forfeit some or all of your damage deposit and could possibly create additional charges.

Rental of the Forman Center is under the supervision of the Forman Park District Board, a non-profit municipal committee.  We reserve the right to refuse service at our discretion.

Most of those who care for the Forman Center do so as volunteers.  Please treat them accordingly.

The rental fee is due on the date indicated on your invoice.  Your date will not be considered reserved until the fee is paid, and will be open to rent to others.

There is a $100 damage deposit required for certain types of events.  These include, but are not limited to, wedding receptions and birthday parties.  The damage deposit will be returned upon inspection if no damage is detected.

Rental rates are based on the actual hours you expect to be in the facility, which includes set up as well as clean-up time.  If you pay the daily rental fee for the gym you may have until 10:00 am the following day to clean up.  There may be rare circumstances where this concession is not possible due to other events.

The kitchen has two stoves and a refrigerator that are available for your use if you rent the kitchen/cafeteria.  There is also a refrigerator in the gym.  The stage, upstairs area and other rooms are off limits.

It is the renter's responsibility to see that the Center is cleaned up inside and out after use.  If we have to hire help to clean, the renter will be billed at the rate of $15.00 per hour.  Help can be hired if you so desire.

We do give out a key for certain events.  The caretaker for the center is Gerry and he can be reached at (309)968-7510.  There is a key drop off box around the corner in the first room to your right as you com in the main entrance.  It is in everyone's best interest if you meet with someone to show you around.

What supplies are furnished?

Unfortunately equipment is hard to keep in a public building where there is no full-time supervisor.  There are two closets for brooms, mops, toilet paper, garbage bags, and paper towels.  The closets are located by the restrooms in the gym.  Any kitchen supplies - towels, wash cloths, soap, pans, table service, coolers, etc, must be provided by the renter.

How many chairs and tables?

The center has chairs and tables to seat approximately 350 people.  This includes 35 round tables 5 feet in diameter which can seat 8 each.  We have 10 - 8 foot rectangular tables and 5 - 6foot tables.  The renter must provide more if needed.  Our tables and chairs must never leave the center.

What are some common damages?

1.  The Forman Center is a smoke-free building.  Smoking is not allowed anywhere in the building.
2.  Late cancellation.
3.  Painted surfaces damaged by tape on the walls.
4.  Leaky ice tubs on the gym floor.
5.  Building not clean.

The renter is responsible for the expenses of any and all damages to the building and claims against the Forman Center as a result of your event.  That includes noise complaints and injuries.  The village has rules on music volume.  Please be considerate of our neighbors.

Can alcoholic beverages be served at our event?

For events where alcoholic beverages are served the renter must provide evidence of liquor liability insurance prior to their event.  Typically, if you are serving alcohol and there is no charge to your guests your homeowner's policy will provide coverage.  Check with your insurance company.

If you plan on having a cash bar, or if your the alcohol is provided by a caterer for a charge, the provider will have to provide you with the evidence of coverage, and there is special licensing required by the town board.
